Ulcerative colitis: grading of severity according to riley

    • To grade macroscopic and microscopic rectal appearance in a trial that compared mesalazine and sulfasalazine in patients with ulcerative colitis
    • Severity of mucosal changes is recorded at three sites within the rectum
    • In the lowest 5 cm, between 5 and 10 cm and above 10 cm with reference to the anal margin
    • A rectal biopsy is taken from the anterior rectal wall between 5 and 10 cm
    • Grades 0 and 1 are considered endoscopic remission
    • Active disease is considered grade II-IV
